WAIVER OF CLAIM: I hereby submit my application for registration in MYFEST26! Tournament. I waive all claims against any persons, schools, or associations connected with the Salvation Army, SGRO Martial Arts, and David Sgro. I assume full responsibility for any injuries I may sustain, as well as for all my actions in connection with this tournament. I agree to obey all tournament rules and regulations. I understand that foam-type headgear, hand and foot pads, and mouthguards are required for all sparring divisions. A groin cup is recommended for males.

I understand that in the event of injury, tournament staff may seek emergency medical treatment on my behalf (or on behalf of the minor competitor named above) if I am unable to consent at that time. I also grant permission for photos or video taken during the tournament to be used for MYFEST26! and SGRO Martial Arts promotional purposes, unless the site owner removes this line.

If under 18 years, a parent or legal guardian must also sign below.
